Numerous post-mortem and seized-sample reports confirm involvement in fatal overdoses; naloxone reverses toxicity but may require repeated dosing because of longer half-life than naloxone. Strong histamine release and chest-wall rigidity have been described with analogous compounds. Always employ test-dose strategy and never use alone - respiratory depression can appear abruptly during or immediately after peak. Fentanyl testing strips are unlikely to detect 4Cl-iBF reliably.
