3-Hydroxyphenazepam is a potent designer benzodiazepine and active metabolite of phenazepam and cinazepam. It is 5-10 times more potent than diazepam and has diminished muscle relaxant properties compared to phenazepam while retaining strong sedative and anxiolytic effects. Recreational doses carry high risk of blackouts, amnesia, and respiratory depression, especially when combined with other CNS depressants. Due to its long half-life and potency, volumetric dosing is strongly recommended. Tolerance and dependence develop rapidly with repeated use, and abrupt cessation can cause life-threatening withdrawal seizures.
